Athletic Clearance needed to tryout
Everyone needs to be cleared before October 15th.
Athletic Clearance
All forms must be uploaded through https://athleticclearance.fhsaahome.org/ DO NOT SEND THEM TO RE.
FHSAA Clearance Registration - You will be asked to upload the completed physical form (EL-2) and the NFHS/ FHSAA video certificates, so we recommend you have those completed before starting this process. [Forms and video links below]
- Watch the tutorial video “See How it Works” or follow the directions below. If at any time you run into a problem you will see a yellow “help” sign at the bottom right corner of the page.
- The 2021-22 FHSAA clearance registration process is now live. Please be sure to select the 2021-22 athletic year.
Below are forms you will need to upload:
Physical Form for your Doctor (FHSAA-EL2) & State REquired videos:
- You can download and print a copy of the FHSAA- EL2 to be completed by your child’s physician. This form will need to be uploaded during the clearance process.
- Your child will also need to complete the following NFHS/ FHSAA required courses AND upload the completion certificates. Instructions regarding form completion are here.
